Birth Registrations and Certificates
Birth Registrations and Certificates are delivered by Service Ontario.
For children born on or after December 10, 2007, parents may now complete the Statement of Live Birth on-line and electronically submit it directly to ServiceOntario's Office of the Registrar General.
To obtain an Ontario Birth Certificate:
- Access an application form from the Province of Ontario's website, or in person at the Township of Perth East.

Death Registrations
A death registration is a permanent, legal record of death. A funeral director or family member is authorized to complete the statement of death once a medical certificate of death stating the cause of death is completed by a physician or the coroner. The funeral director, on behalf of the deceased person's family, usually initiates this process, ensuring that both forms are complete and submitted to the Township of Perth East (local registrar) for the purpose of acquiring a burial permit.
